WHAT IS ADULT DAY HEALTH CARE CENTER?
• Adult Day Health Care Center is a Community Based Adult Services (CBAS) program that provides services to older adults and adults with chronic medical illnesses, cognitive decline, or mental health conditions and/or disabilities.
• The ADHC Center is designed to meet the needs of individuals who are frail, physically limited, or isolated. Transportation is made available door to door to pick-up participants and return them safely home.
• ADHC services are also of great benefit to family members. Caregivers can relax knowing that their loved ones are in a safe and supportive environment under the supervision of the ADHC healthcare professionals.

Healthcare Services: Skilled nursing care, medication management, health monitoring, and assistance with activities of daily living.
Therapeutic Activities: Engaging programs designed to stimulate physical, cognitive, and emotional well-being, including art therapy, games, music therapy, exercise classes, and more.
Nutritious Meals: Delicious and well-balanced meals and snacks accommodating dietary restrictions and preferences.
Transportation: Convenient transportation services to and from our center, ensuring safe and reliable access for our participants.
YOU MAY BE ELIGIBLE FOR SERVICES IF YOU
• 18 years or older
• Individuals who live alone and have no family or caregiver available to provide sufficient and necessary care or supervision.
• Individuals who may have physical, cognitive, or mental health challenges that require assistance or supervision during the day. This may include individuals with chronic illnesses, and disabilities.
• Each individual will require an assessment or evaluation to determine if the individual's needs and goals align with the services provided.
Adult Day Health Care Center Services are covered by Medi-Cal and most insurances.
Our staff will help determine their eligibility.
